Passport Appointment: First-time Applicants and Children (Non-Renewals)

Walk in services are based on availability and individuals are expected to arrive at least 30 minutes prior to closing.  Please Note:  Each individual passport application can be expected to take 15 minutes.

Bring the following with you when applying for a passport in our office:

  • Completed DS-11
  • Proof of U.S. citizenship
  • Identification
  • Photos (available at above locations for $15.00 fee)
  • Fee payment
  • Social Security number

Both parents must be present when making application for a minor under the age of 16 and have a valid ID such as driver license or passport.  A minor under the age of 16 that has previously had a US Passport must submit the certified birth certificate and passport when application is made to renew a passport.  Applicants ages 16 and 17 must have one parent present to sign the application and show valid ID.

Proof of Citizenship
Bring any one of the following:
  • Expired U.S. passport
  • Naturalization certificate or certificate of citizenship or report of birth abroad
  • Certified copy of birth certificate if born in the United States; it must show:
    • Name of the child (including given name)
    • Date of birth
    • Place of birth
    • Date the certificate was filed in the registrar's office - date must be within one year of birth
    • Signature of registrar and authorized seal of that office
    • Parents: the full name(s) of the parent(s) of the bearer are required for all applicants 

Important: Hospital birth certificates and baptismal certificates are not acceptable.

Bring any one of the following:
  • A valid driver's license (not a temporary or learner's permit)
  • Naturalization certificate or certificate of citizenship
  • Official military ID (current)
  • Valid U.S. passport

Additional ID will be required if using the following types, possibly an affidavit of Identifying Witness form while in our office. (Please check with a license representative in our office concerning these).
  • State picture ID card
  • Government ID card (federal, state, or municipal); or work ID with picture
  • Pilot's license

A photo is required.  You may have the photos taken here for $15 or bring in your own photos which must meet these requirements:
  • Size: 2 inches by 2 inches; image size not less than 1 inch nor more than 1-3/8 inches from the bottom of the chin to the top of the head, including hair
  • Color photos with a white or off white background
  • Subject must be looking right at camera - no side views

The passport fee must be paid by check or money order payable to the U.S. Department of State. A separate payment must be sent with each application.

The execution fee of $25 per passport application may be paid by check or money order payable to Washington County or cash. If more than one application is being processed the execution fees and photo fees can be combined on one check.
