Passport
Visit Washington County Service Centers to:
- Pick up passport forms or download printable forms from the U.S. Department of State.
- Get passport photos taken at all four locations.
- 2 color photos.
- $15 per person.
- Form of payment accepted are: cash, check or money order payable to "Washington County," and credit/debit card (for an additional 2.49% fee).
- Schedule an appointment online to take passport photos.

Please be sure to bring the following items when you apply for a passport. If you do not have all the required items, your application cannot be processed, and your appointment will be rescheduled. All applicants (including minors) must be present at the appointment. Both parents and/or legal guardians of children under age 16 must appear in person. Visit travel.state.gov for allowable exceptions.

- 1st Payment: payable to U.S. Department of State

Completed DS-11 Passport Application
- Use black ink only.
- Do not use white-out.
- Do NOT sign the form.
- Printable applications available online at Form DS-11 (PDF) (printed single-sided).
- Application forms are also available at our office, but please arrive early for your appointment to ensure enough time to complete the form.
Proof of U.S. Citizenship
- Must show an original or certified copy.
- Must provide a black and white photocopy of the document.

- Must meet requirements set by U.S. Department of State.

Apply in person when:
- You are applying for a passport for the first time.
- You lost your passport book or card.
- You are renewing a valid 5-year passport.
- Your passport was issued over 15 years ago.
- Your passport has been damaged.

What to bring:

- For an additional $15, Washington County Service Centers can take your passport photo. Or, bring a passport photo that meets Department of State requirements.
How to Pay:
- Review Passport Fees and bring 2 separate payments.
- One separate check or money order per application payable to "U.S. Department of State."
- One separate payment of $35 per application for processing fee.

You renew by mail, if:
- You still have a passport.
- You were 16 years old or older when your passport was issued.
- Your passport was issued 15 years ago or less (issue date on passport).
- Your passport was valid for 10 years.
- Your passport is undamaged.
- Your passport was issued in your current name, or you can provide name change document.
