Mission: The Heading Home Washington Collaborative is a network of various organizations and concerned individuals devoted to the issue of accessibility to affordable housing and elimination of homelessness for individuals and families.

Vision: The Heading Home Washington Collaborative creates solutions so that everyone can access and maintain affordable housing.

Coordinated Access for Homeless Households

Coordinated Access is a new resource to help those who are homeless (or within days of eviction) access appropriate housing and services. Please keep in mind that, due to limited resources, we may not be able to assist you in finding an immediate resource.

If you are currently receiving case management services of any kind from Washington County, please start by contacting your county case manager.

Coordinated Access services are provided by three different agencies, depending on the household needing help: single adults, families with children, or unaccompanied youth.
  • Youth under age 22: Call Canvas Health at 651-251-5214
  • Families with children: Call St. Andrews Community Resource Center at 651-762-9124
  • Single adults/couples: Call Washington County Community Services at 651-430-6488
