Mission: The Heading Home Washington Collaborative is a network of various organizations and concerned individuals devoted to the issue of accessibility to affordable housing and elimination of homelessness for individuals and families.

Vision: The Heading Home Washington Collaborative creates solutions so that everyone can access and maintain affordable housing.

Coordinated Entry for Homeless Households

Coordinated Entry is a new resource to help those who are homeless (or within days of eviction) access appropriate housing and services. Please keep in mind that, due to limited resources, we may not be able to assist you in finding an immediate resource.

If you are currently receiving case management services of any kind from Washington County, please start by contacting your county case manager.

Coordinated Entry services are provided by three different agencies, depending on the household needing help: single adults, families with children, or unaccompanied youth.
  • Youth under age 24: Visit Youth Service Network's website:  www.ysnmn.org
