Health Improvement Plans
Below are Health Improvement Plan summaries for the 12 key health and environment problems identified in the 2008 Community Health Assessment. These plans represent summary action steps, goals, and performance measures for a two-year period, from 2009-2011. The plans will be reviewed and updated in 2011, then will continue through December of 2013.
