Long Term Care Consultation (LTCC)
A service completed upon request at no cost, to Minnesotans regardless of income, social status or age. The LTCC is an assessment completed by a social worker and/or a public health nurse in the person's home. During the LTCC, the person is asked questions about their general physical and mental health, memory, environment, their ability to complete day-to-day activities and the amount of care that they receive from family or other supports. Family members, friends and other caregivers are encouraged to attend as they may be able to provide additional input. Resource information and recommendations for home and community based services are made according to the persons specific needs, as identified through the assessment process. An LTCC is typically about 2 hours in length.

Relocation Service Coordination (RSC)
Specialized case management for persons in a nursing home or other facility who are requesting to relocate from their current living situation to the community. This service is only for people who are on Medical Assistance and who have had a Long Term Care Consultation. RSC provides case management which assists the consumer in finding appropriate housing and making referrals to services to meet the individual's needs in the community.

Alternative Care (AC) Grant Waiver and Elderly Waiver (EW) Programs
Programs for persons who are 65 years of age or older, have had a Long Term Care Consultation to determine eligibility, and are at risk of nursing facility placement.
The goal of the AC and EW programs is to assist seniors, by providing payment for supportive services, so they may continue to live independently in the community.
To be eligible for the Alternative Care (AC) Grant Waiver program, consumers must meet financial eligibility criteria specific to the AC Grant Waiver. Persons on the Elderly Waiver must be on or eligible for Medical Assistance.
Both programs can pay for services such as but not limited to: case management, homemaking, home health aides, skilled nurse visits, home delivered meals, adult day programs, assisted living cares, home modifications and special supplies and equipment.

Community Alternatives for Disabled Individuals (CADI) Waiver and Traumatic Brain Injury Programs
Programs for persons who are 64 years of age or younger, have a certificate of disability, are on or eligible for Medical Assistance, have had a Long Term Care Consultation to determine eligibility, and are at risk of nursing facility placement. In addition for the Traumatic Brain Injury (TBI) Waiver, the person must have a diagnosis of a traumatic or acquired brain injury and must be experiencing deficits as a result of this diagnosis.
The goal of the CADI and the TBI waiver is to increase an individual's independence in the community through the payment and use of home and community based services.

Community Alternative Care (CAC) Program
Programs for chronically ill persons who are 64 years of age or younger, have a certificate of disability, are on or eligible for Medical Assistance, are determined to need hospital level of care, and choose care in the community. A home care plan must be developed that assures the health and safety of the applicant and the cost of this plan must be equal to or less than MA pays for each individual's hospital care.
The goal of the CAC program is to enable individual's with chronic health conditions or disabilities to safely live in a less restrictive environment as an alternative to a medical institution.
In addition to the services provided through Medical Assistance, there are a wide range of additional services available under the CAC, CADI and TBI programs. Additional services include but are not limited to: case management, respite care, homemaking, home health aides, adult day programs, independent living skill instruction, family counseling and training, home modifications, adaptive equipment, assisted living cares and home delivered meals. In addition, behavioral programming is available under the TBI program.

Personal Care Assistance (PCA)
Program provides services to individuals of any age who require additional care to meet their day-to-day needs. Participants must be on Medical Assistance. The recipient must be able to direct their own care or have a responsible party who is able to do so on their behalf. A county public health nurse will complete a PCA assessment to determine if PCA services are appropriate to determine the amount of time needed to assist the care receiver.
