Service and License Centers

Passports

Applications are requested to arrive at least half-hour before closing to assure ample processing time.

Information for Passport Applications

Passports are available at the following locations in Washington County:


Service Center in Cottage Grove
Service Desk in Forest Lake - no passport applications after 5 p.m. on Monday and Wednesday or Saturdays.

Monday - Friday
8 a.m. - 5 p.m.

License Center in Stillwater
Service Desk in Woodbury

Monday and Wednesday
8 a.m. - 6 p.m.
Tuesday, Thursday and Friday
8 a.m. - 5 p.m.
Saturday 8 a.m. - 12 noon

Bring the following with you when applying for a passport in our office.

1. PROOF OF U.S. CITIZENSHIP (bring one document)

NOTE: All passport applicants must be present at the time of application.
A. Expired U.S. Passport
B. Naturalization Certificate, or Certificate of Citizenship or Report of Birth Abroad.
C. Certified copy of Birth Certificate if born in the United States; it must show:
1. name of the child (including given name)
2. date of birth
3. place of birth
4. date the certificate was filed in the registrar's office - date must be within one year of birth
5. signature of registrar and authorized seal of that office

IMPORTANT: Hospital birth certificates and baptismal certificates are not acceptable!

2. IDENTIFICATION (bring one)

A. A valid Driver's License (not a temporary or Learner's Permit)
B. Naturalization Certificate or Certificate of Citizenship
C. Official Military I.D. (current)
D. Valid U.S. Passport

Additional I.D. will be required if using the following types E through G. Possibly an Affidavit of "Identifying Witness" form while in our office. (please check with a license representative in our office concerning these).

E. State picture I.D. card
F. Government I.D. card (Federal, State or Municipal); or work I.D. with picture
G. Pilot's License
H. Minors
1. All minors must have their own passports and be present at the time the application is processed. Both parents must be present with any child under the age of 16 with their valid Driver's License or Passport and it is required that each parent sign the application in the presence of the License Center staff when the application is processed. If the applicant is under the age of 16 and previously had a passport, the passport and a certified birth certificate will be necessary to renew that passport. Please call for further instructions on what procedures to follow if one of the parents is not able to be present.
2. Minors between 16 and 18 must sign the application and may use their own valid driver's license or permit. BUT parental signature is still required and the parent's I.D. document must be used on the application.

2. Photos

2 photos are required. You may have the photos taken here (2 for $15) or bring in your own photos which must meet these requirements.

A. They must be duplicates.
B. Size: 2 inches by 2 inches; image size not less than 1 inch nor more than 1-3/8 inches from the bottom of the chin to the top of the head, including hair.
C. Two identical color photos with a white or off white background.
D. Subject must be looking right at camera - no side views.

 The passport fee must be paid by check or money order payable to the U.S. Department of State. A separate payment must be sent with each application.

The execution fee of $25 per passport application may be paid by check or money order payable to Washington County or cash. If more than one application is being processed the execution fees and photo fees can be combined on one check.

 

5. SOCIAL SECURITY NUMBER - Please have this with you when you apply.

NOTE: Apply for your passport early to ensure that you receive it in time. Allow up to six weeks to receive your passport using the routine renewal service. If you have any questions about needing your passport sooner than six weeks, please call ahead for help.

Washington County does not discriminate on the basis of race, color, national origin, sex, religion, age and handicapped status in employment or the provision of services.


PASSPORT RENEWALS

A passport applicant may renew through the mail provided you meet the following criteria:

1. You can submit your most recent passport.
2. You were at least 16 years old when your most recent passport was issued.
3. You were issued your most recent passport less than 15years ago.
4. Your name is the same as on your most recent passport; or your name was changed through marriage or court order and you can submit proper original documentation (certified copies) to reflect your name change.

A. You must then complete the renewal form (DS-82).
B. You need to submit two new identical pictures as specified on the DS-82 form.
C. You need to submit a check or money order for $110 to Passport services.

Enclose all of the above items (A-C) in an envelope together with your most recent passport and mail to:

Please allow the same amount of time as for original applications.
If you will need your passport in less than six weeks, you may be eligible to expedite your passport renewal.
Please inquire at the service counter about this procedure.

PASSPORT CARDS

As of Feb. 1, 2008, the State Department will begin accepting applications for passport cards as alternatives to passports. The passcard is not valid for air travel to Canada and Mexico. This wallet-sized card will be issued to the applicants by late spring. You may apply for a passport book and card at the same time and pay the one execution fee.
