Anyone wishing to receive a permit for a new or replacement septic system must complete an application form from the Department of Public Health and Environment. The following information items are included on the application form and may be necessary for final approval of your application:
- The legal description and parcel identification number, especially if this is a new subdivision or minor subdivision.
- Your name, address and phone number.
- The owner's name address and phone number, if different from yours.
- Whether this a new home or business, or an existing one.
- Type of system being installed, i.e., mound or drainfield.
- A site approval letter if previously approved.
- Percolation test results.
- Soil boring logs.
- Site plan (drawn to scale) showing building locations, lot lines, percolation test holes, soil boring holes, proposed location of system and well.
- One copy of the system design.
- One copy of the final building plan.
