Food Safety Complaint Process


How do I make a formal complaint about a restaurant?


Call the department's main phone number at 651-430-6655. Indicate which community the establishment is located in and you will be transferred to the inspector assigned to that community.

How do I report a foodborne illness?


If you suspect that you or someone you know has become ill due to food you ate, contact the department's main phone number at 651-430-6655. You will be connected with an environmental health specialist, or the receptionist will take your name and phone number and you will be contacted shortly. As an alternative, you can also report your illness to the Minnesota Department of Health Foodborne Illness Hotline at 1-877-FOOD-ILL (1-877-366-3455).

The interview for a suspected case of foodborne illness takes approximately 10-15 minutes. Because the food you last ate may not be the cause of the illness, the environmental health specialist will ask for a 3-4 day food history. Before calling, try to remember all of the beverages and food you have eaten 3-4 days prior to the onset of illness. This is easier than it sounds. The environmental health specialist will ask about home prepared meals, as well as your dining out experience.
